A number of staff from Altnagelvin Hospital are staging a picket outside the Derry hospital.
They are among more 4,000 NHS (National Health Service) staff throughout the North who are staging a three-and-a-half-hour strike over pay.
Members of the unions, Unite and GMB, walked out at 11.00am and are due to resume work at 2.30pm.
Urgent and emergency services are not being targeted, unions said.
Unions want a one per cent pay rise to be implemented for all staff, but the NHS said this would cost too much.
Among those taking strike are office workers, laboratory and pharmacy staff, community nurses and catering staff.
